EE WAS ADDING THE MOLD EYEBOLTS TO THE TOP OF THE TOOL (MOLD) [REDACTED] WHEN HE WAS PRESSING THE BUTTON ON THE CRANE REMOTE CONTROL THE CRANE STARTED TO MOVE UP. EMPLOYEE STATES THAT THE CRANE MOVED BY ITSELF. THERE WAS TOO MUCH TENSION ON THE CRANE AND THE RELIEF PIN FOR THE CRANE EYEBOLT BROKE. THE CHAIN HIT THE EMPLOYEE ACROSS THE MOUTH AND LEFT EYEBROW
