On [REDACTED] reported to SDO that while she was in the women 's restroom across from [REDACTED] in the first stall; the lock did not work and she was accidentally hit on the top left side of her forehead when the stall door was opened by another employee. Initially she stated that she thought that she was OK then her head starting pounding. She requested Tylenol but decided to go home instead.
