Loading a trailer with a forklift

What happened

Driver was loading a trailer at [REDACTED] location when the doors strap got caught on a bolt on the mast causing the door to collapse. There was a hole on the end of the strap hanging down that got perfectly snagged on the lift mast. No driver error was detected. Associate was hit on the head with one of the door wheels and was written out of work for 2 days.
