Employee removed the cover to a belt on a high pressure pump. He turned the pump off but did not wait for the equipment to be fully de-energized and he did not perform lockout tagout.

Employee removed the cover to a belt on a high pressure pump he leaned forward and his fingers were caught on the belt resulting in two fractures and a laceration. Employee was taken to the hospital and returned to work after 5 days.
