THERE WAS A PALLET ON THE FLOOR BEHIND THE EMPLOYEE THAT BOXES WERE SUPPOSED TO BE STACKED ON. THE EMPLOYEE DID NOT KNOW THE PALLET WAS THERE SO WHEN TURNED AROUND TRIPPED ON THE CORNER OF THE PALLET LOST HER BALANCE AND FELL TO THE FLOOR.
