Before the incident

Employee was throwing away scrap pallets that were lying around the parking lot area into a large roll off.

What happened

Employee used a telehandler to place an oversized pallet into the roll off. It did not fall in so he went into the shop and grabbed a sawzall and then climbed up onto the roll off by using his buggy seat and the rail on the side of the roll off. He was standing on he pallet and when he made a cut it fell into the roll off and took him with it. He used his radio to call for help.
