Ground level cleaning out dirt from a footing

What happened

employee was working in the south east ground level cleaning out dirt from a footing as he was going down the ladder into the footing his left foot slipped causing him to lose balance and fall towards his right. As he tried to catch himself and regain balance his right hand slipped in between the rebar form causing it to bend forward with all his weight on top of it.
