What happened

the toolbox talk ended he returned to the sea container to retrieve his backpack. When he pulled his backpack he also pulled the concrete vibrator that was sitting on the top shelf causing it to fall striking his left foot. This resulted in the injured worker having multiple fractures in his left foot.
