Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
a crushing hazard:
a) Steel Producting Division, Open Hearth Department, Chaning Floor
#6 Furnace - The Warner Swasey hydraulic excavators, company #62 and
#64, were not provided with an audile alarm to warn employees of
backward movement when the view to the rear was obstructed, September
23, 1989.
Among others, one feasible and acceptable abatement method is to install
an audible alarm or use a spotter when view to rear is obstructed.
