Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
Crushing injuries due to the inadvertant activation or unintended
motion of the Grand-O-Matic 360B track mounted, remote controlled
digger (Serial No. 04188969) in use at the No. 2 blast furnace
on November 16, 1989. The pendant control box for the Grand-O-Matic
machine was not clearly marked for identification of functions.
Among other methods, one feasible and acceptable method to correct this
hazard is to legibly and durably identify each control button or switch
to identify its function.
