Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the explosion of molten metal from furnaces that were not adequately protected against such explosions. CASTING DEPARTMENT: Worker's were exposed to injury while operating a permanent mold casting furnaces in which the company had not adequately perform a hazard analysis of the casting process that fully identified, evaluated or controlled known hazards. On or about 2/23/99, a worker was injured on company furnace number 4, when molten metal came into contact with water, causing an explosion on the furnace he was casting. Amongst others, some feasible means of abatement for protecting the worker would include Increasing the Caster's Distance from the furnace or providing Protective Barriers such as screens when distance is not feasible.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that hoists with deficiencies were inadequately repaired and/or were continued to be used. These include but are not limited to the following: Casting and Metal Storage Departments P&H 4000 lb hoist, Company # 362 a)The hoisting limit switch was inadequate in that the limit switch did not function properly and required additional weight in order to keep the switch in the on position. b)Buttons on the pendant control were not marked so as to identify it's function. Hoist (No Name), Company #365 c)The hoist had been tagged out of service since 3/3/99, but had not been locked out. Rod Mill Department P&H Hoist, Company #201 d)The pendant control box was not labeled to identify the function of the buttons. e)The hoist was found to have excessive wire rope on the drum. The rope was observed to be loosely wrapped with rope overlaying each other as well as pieces of rope sticking out, well away from the drum. P&H Hoist, Company #228 f)The hoist was observed to have seven (7) wraps on the drum. Two (2) wraps were observed on one side of the drum while the other five (5) wraps were on the other side of the drum. Part of the wire rope laid across the ridges of the grooves on the drum
