Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment, and a place of employment which were free from
recognized hazards
that were causing, or likely to cause death or serious physical harm to
employees in that: an
employee was exposed to a tipping hazard while working from a scissor
lift:
(a) Exterior, southwest corner of 7316 W. Fairview Ave: On April 26,
2006 and times prior thereto, employee(s) engaged in demolition
activities, were exposed to a tipping hazard, when they did not follow
the manufacturers instructions and used the tilt alarm as a level
indicator for a Genie scissor lift, model GS2032, s/n 37026.
(b)Exterior, southwest corner of 7316 W. Fairview Ave: On April 26,
2006 an employee engaged in demolition activities, was exposed to a
tipping hazard, while working from a Genie scissor lift, model
GS2032, s/n 37026, which was raised approximately 20 feet high and
positioned on a single sheet of plywood, on top of a gravel surface, that
was not firm or level.
(c)Exterior, southwest corner of 7316 W. Fairview Ave: On April 26,
2006 an employee engaged in demolition activities, was exposed to a
tipping hazard, when a load (angle iron) was placed outside the
platform perimeter of a Genie scissor lift, model GS2032, s/n 37026,
while the platform was fully extended and raised 20 feet.
NOTE 1) A feasible and acceptable abatement method to correct these
hazards is to use a
scissor lift designed specifically for the area it is to be used in.  A
rough terrain scissor lift
would have been an acceptable machine to use in this situation.
NOTE 2) Manually propelled elevating aerial platform (scissor lift)
requirements are covered
under American National Standards Institute (ANSI) A92.3.
Note: Abatement certification is required for this item.
