Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls from an elevation: (a) Plumbing Shop, Pipe Threading Machine Area, South End - the Bauer plastic reinforced (fiberglass) 6 foot step ladder was damaged on the side rail at the bottom portion of the step ladder exposing employees to a fall hazard. The ladder was last used by employees on or about March 16, 2007. Among other methods, one feasible and acceptable method to correct this hazard includes but is not limited to the following: In accordance with ANSI A14.5 - 2000 - American National Standard Institute for Ladders - Portable Reinforced Plastic - Safety Requirements: Section 9.4.1 "Ladder shall be inspected before each use and where structural damage or other hazardous defect is found, the ladder shall be taken out of service and either discarded or repaired by a competent mechanic." Section 9.4.2." Broken and bent ladders shall be marked and taken out of service until they are repaired by a competent person or destroyed in such a manner as to render them useless. No abatement certification or documentation is required for this item.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards: (a) Fiberglass Department, Small Spray Booth, South End - the six automatic water sprinkler heads inside the Binks dry filter spray booth were protected by polyethylene (plastic) bags to protect the sprinkler heads from overspray residue. The plastic bags would melt in event of a fire causing the sprinkler heads not to operate quickly. (b) Fiberglass Department, Large Spray Booth, North End - the six automatic water sprinkler heads inside the Binks dry filter spray booth were protected by polyethylene (plastic) bags to protect the sprinkler heads from overspray residue. The plastic bags would melt in event of a fire causing the sprinkler heads not to operate quickly. Among other methods, one feasible and acceptable method to correct this hazard includes but is not limited to the following: In accordance with National Fire Protection Association 33 Standard for Spray Application Using Flammable or Combustible Materials - 2007 edition, Section 9.4.7.1 " Sprinklers shall be permitted to be covered only by cellophane bags having a thickness of 0.08 mm or less or by thin paper bags. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a) Mechanical Department, Zone 3, East End - the Six Flags mechanical "below the hook" lifting device being used on the blue CM electric chain hoist was not marked with the proper rated load and lifter weight if over 100 pounds. Among other methods, one feasible and acceptable method to correct this hazard includes but is not limited to the following: In accordance with American National Standard Institute for Below the Hook Lifting Devices B30.20-2006, Section 20-1.2.1 "The rated load of the lifting device shall be legibly marked on the main structure or on a tag attached to where it is visible." No abatement certification or documentation is required for this item.
