employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard:
a) On or about 12/9/08 and 3/24/09, employees were exposed to a fall
hazard of 4 feet
and greater while harvesting mushrooms from a slippery one-inch working
platform.
b) On or about 3/24/09 employees were exposed to a fall hazard of
approximately 6 feet
while walking along unguarded openings.
c) On or about 12/9/08, employees were exposed to a fall hazard of
approximately 6 feet
and higher while performing activities of watering, spraying the mushrooms
beds while
in almost total darkness.
d) On or about 12/9/08 and 3/24/09 employees were exposed to fall and
ergonomic
hazards while accessing and working all day, from a platform measured at
approximately
one inch wide and 54 inches high, without the benefit of a ladder.
An acceptable means of abatement is to provide a working and walking
platform with
guardrails, where employee can stand on their two feet to harvest the
mushrooms, water
and spray the mushroom beds. Provide safe access to these platforms
through the use of
ladders. Provide guardrails to floor openings.  Provide handrails to the
stairs.  Improve
lighting, and establish a system where employees can lower the mushrooms
boxes safely.
