a) Maintenance Warehouse -
A cylinder of compressed halon gas discharged its contents while being
moved by an employee.
The valve caps were not on the cylinder while it was being moved. On or
about 10/28/09.
b) Maintenance Warehouse -
A cylinder of compressed halon gas was moved by grasping the valve at the
top of the cylinder
and rolling its bottom edge on the ground. On or about 10/28/09.
c) Maintenance Warehouse - A hand truck or other similar device was not
used while moving
a cylinder of compressed halon gas. On or about 10/28/09.
ABATEMENT NOTE: FEASIBLE ABATEMENT METHODS WOULD BE TO: 1) ENSURE
THAT COMPRESSED GAS CYLINDERS ARE NOT MOVED UNLESS ALL CAPS AND
OTHER SAFETY DEVICES ARE IN PLACE. 2) ENSURE THAT COMPRESSED GAS
CYLINDERS ARE NOT MOVED BY "WALKING' THEM ON THE GROUND. 3) ENSURE
THAT COMPRESSED GAS CYLINDERS ARE SECURED TO A HAND TRUCK OR
SIMILAR DEVICE WHILE BEING TRANSPORTED.
NOTE:  BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED
IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR
DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED
BY 29 CFR 1903.19.
