29 CFR 1910.23(b)(8): The employer did not ensure that ladders are used only for the purposes for which they were designed:  a) On or about 01/25/2024, throughout the store; employees were exposed to fall hazards when the employer did not ensure a Werner 8-foot Type I fiberglass step ladder model #6008 was used in accordance with manufacturer's requirements. Employees used the top two steps of the ladder to access merchandise on an approximate 9-foot 5-inch Overhead Storage Platform (OSP).  b) On or about 01/22/2024, in the back storage/breakroom area;  employees were exposed to fall hazards when the employer did not ensure a Werner 8-foot Type I fiberglass step ladder model #6008 was used in accordance with manufacturer's requirements. Employees lacked sufficient space to fully extend the ladder to reach toilet paper and paper towels from the mezzanine above the bathroom. As a result, they used the ladder in its closed position, leaning it against the wall.  c) On or about 01/25/2024, in the back storage/breakroom area;  employees were exposed to fall hazards when the employer did not ensure a Werner 8-foot Type I fiberglass step ladder model #6008 was used in accordance with manufacturer's requirements. Employees lacked sufficient space to fully extend the ladder to reach a Jumbo Card rack from the mezzanine above the bathroom. As a result, they used the ladder with spreaders that were not fully extended and locked.  ABATEMENT DOCUMENTATION REQUIRED.
