of the east slip were exposed to a struck-by hazard due to a falling boom
and load
from a Manitowoc crawler crane Model Number: 4500, Serial Number: 4503.
Among other methods, one feasible and acceptable method to correct this
hazard
would be to inspect, in accordance with the manufacturer's
recommendations, the
worm shaft and gear for damage and excessive wear each time the oil for the
boom hoist is changed or every 2000 hours or six months, whichever comes
first.
If either part is worn excessively or damaged, replace both parts.
