Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to burn and struck by hazards due to the improper set up and maintenance of the Geronimo line:    a) On or about October 26, 2011, and at times prior, employees were exposed to injury while working on a drill rig in that the shackles on the Geronimo Line had several broken wires.    b) On or about October 26, 2011, and at times prior, employees were exposed to injury where the Blooey Line had been installed in the path of the Geronimo line.

29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15-, 20-, and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel shall have ground-fault circuit-interrupter protection for personnel.  Note 1 to paragraph (b)(3)(ii)(A) of this section: A cord connector on an extension cord set is considered to be a receptacle outlet if the cord set is used for temporary electric power.  Note 2 to paragraph (b)(3)(ii)(A) of this section: Cord sets and devices incorporating the required ground-fault circuit-interrupter that are connected to the receptacle closest to the source of power are acceptable forms of protection.    a) On or about October 26, 2011, and at times prior, employees were exposed to injury in that, employees were exposed to injury in that, the companyman and toolpusher trailers were not provided with GFCI.

Precision Drilling Company LP, was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.151(c) which was contained in OSHA inspection 312292675 citation number 1 item 1 issued on 09/30/2009, and became a final order on 10/22/2009 with respect to a workplace located at Pats Ridge Road, Heyner, PA 17738.
