Packing materials and wood scraps had been allowed to accumulate in working areas, exposing employees to slip and trip hazards.    b) Sign Source USA, Inc. - Lima, Ohio: On or about January 5, 2012, the metal department, where metal signs are fabricated and welded was not maintained in a clean and orderly condition.  Scrap metal had been allowed to accumulate in working areas around machinery and walkways, cords and hoses were observed across walkways exposing employees to slip/trip and laceration hazards.
