29 CFR 1926.501(b)(14): "Wall openings." Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    a. On or about December 19, 2011, at the above addressed jobsite, employees performing demolition operations were exposed to falls of approximately thirty seven (37) feet, created by the presence of wall openings.    THE AMERICAN DEMOLITION CORPORATION, WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312593908, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 13, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT 320 E. HURON STREET, CHICAGO, IL 60611.
