The extension cord ran up the wall and over the to sample analysis station where it was plugged into another relocatable power tap (daisy chained) that was providing power to the CEM brand loss of moisture process analytical equipment. AMERICAN AEROGEL WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314014747, CITATION NO. 1 , ITEM NO. 2 , AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 1, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 460 BUFFALO ROAD, ROCHESTER, NEW YORK, 14611. 29 CFR 1910.157(g)(4): The employer shall provide the training required in paragraph (g)(3) of this section upon initial assignment to the designated group of employees and at least annually thereafter. a) On or about 3/27/12 throughout the facility; employees designated by the employer to use fire extinguishers on incipient fires were last trained November 16 & 18, 2010 and there have been two fire incidents within the last six months where designated employees were involved.
