29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) On or about 05/12/2020 in the Finishing Department/Cutting Room; where employees were exposed to amputation hazards while operating a radial arm saw on which the sides of the lower exposed portion of the blade were not guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock (insulation material) and remain in contact with stock being cut to give maximum protection possible for the operation being performed. While cutting stock that was one inch thick, the upper blade guard was four inches above the table. Additionally, the saw was not installed so as to cause the cutting head to return gently to the starting position when released by the operator; the saw did not return at all to the starting position.  ABATEMENT DOCUMENTATION REQUIRED
