29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:    (a) On or about May 3, 2012, and at times prior, at the Sheyenne Mini Storage project located at 425 Christianson Drive in West Fargo, North Dakota, employees used a pallet on the forks of an extendable boom forklift as a means of access from the roof of a storage unit building approximately 14 feet in height from the eave to ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) On or about May 3, 2012, and at times prior, at the Sheyenne Mini Storage project located at 425 Christianson Drive in West Fargo, North Dakota, employees were engaged in roofing near unprotected sides and edges of a storage unit building approximately 14 feet in height from the eave to ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) On or about May 3, 2012, and at times prior, employees engaged in sheet metal roofing at Sheyenne Mini Storage at 425 Christianson Drive in West Fargo, North Dakota, used a 12' step ladder to access the roof approximately 14 feet or more above the next lower level and the ladder did not extend past the upper landing to which the ladder was used to gain access.
