29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    The employer did not ensure that employees working near unprotected sides and edges 6 feet above a lower level are protected from falling with a personal fall arrest system.   This violation occurred on or about 5/22/12, north side of building, employee was exposed to a fall hazard of more than 6 feet when painting on a metal structure from the roof canopy of the building without utilizing a fall protection system.

The employer does not ensure that ladders are used for the purpose for which they are designed for.  This violation occurred on or about 5/22/12, north side of building, employee conducting painting activities accessed a roof canopy over the sidewalk with a closed 12 Husky A Frame ladder used as an extension ladder.
