29 CFR 1910.215(a)(4): Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage:       (a) In the maintenance shop on or about May 24, 2012, and at time prior thereto a Craftsman 6 inch Bench Grinder with Leg Stand work rest was 3/8 inch from the abrasive wheel.  The bench grinder was used to grind small metal parts and tools.
