29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listed or labeling:   In the following locations employees were exposed to electrical hazards:  A - Maintenance Shop in this location the following electrical hazards existed           1 - A 4-inch metal receptacle box with knockouts designated for fixed location was wired with a yellow flexible cord with plug and being utilized as an extension cord.          2 - A Yellow relocatable power tap was not used in accordance with instructions included in the listing or labeling. The relocatable power tap had a coffee maker, microwave oven and a water heater/cooler plugged into               it.  B - The Bingo building:  A 4-inch metal receptacle box with knockouts designated for fixed location was wired with a yellow flexible cord with plug and being utilized as an extension cord.
