OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a combustible long chain chlorinated paraffin resin dust while working at or near several indoor dust collectors which were not designed or installed to prevent or minimize employee exposure in the event of a deflagration or other uncontrolled fire event: a. On the first floor in the Southeast corner of building 1 grinding, the #6 Beckert and Hiester dust collector S-012-16 (model: 6-380-36-84S-3G) located inside for baggers 1 and 2 lacked means of 1) explosion protection, 2) fire protection, and 3) deflagration propagation isolation to the upstream top and bottom bagging system via the inlet and collection drum via material discharge. b. On the third floor mezzanine in building 1 grinding, the #2 Air Purification Methods dust collector S-013-16 (model: 14109.6 Super Jet Series; S/N: AB0816-74) located inside for the bottom belt lacked means of 1) explosion protection, 2) fire protection, and 3) deflagration propagation isolation to the upstream bottom belt collection bin via the inlet and product bin via material discharge. c. On the third floor mezzanine in building 1 grinding, the #5 Air Purification Methods dust collector S-011-16 (model: 14109.6 Super Jet Series; S/N: AB0203-90) located inside for the top belt lacked means of 1) explosion protection, 2) fire protection, and 3) deflagration propagation isolation to the upstream top belt collection bin via the inlet and product bin via material discharge. ABATEMENT NOTE: Among other methods, feasible and acceptable abatement methods to correct this hazard include adhering to the following: * NFPA 654, Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids (2006 Edition), Sections 7.13.1.1 and 7.1.2, including, but not limited to: relocating the dust collectors to the outside of building 1; equipping the dust collectors with deflagration vents that are designed to withstand the effects of deflagration and are vented through ducts to the outside; equipping dust collectors with oxidant concentration reduction in accordance with NFPA 69, Standard on Explosion Prevention Systems; equipping dust collectors with deflagration pressure containment in accordance with NFPA 69; equipping dust collectors with deflagration suppression systems in accordance with NFPA 69; diluting with a noncombustible dust to render the mixture noncombustible; or equipping dust collectors with deflagration venting through listed dust retention and flame-arresting devices. * NFPA 654, Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids (2006 Edition), Sections 7.13.1.2, 7.1.3, and Chapter 10, for fire protection of equipment. * NFPA 69, Standard on Explosion Prevention Systems (2008 Edition) and NFPA 654, Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids (2006 Edition), Sections 7.1.4 and 7.1.5, for isolation and deflagration isolation of equipment.

On the first floor bagging area of building 1 grinding, fugitive "Chlorez 700 S" dust accumulation of .798 to .829 inches which had covered the surface on the control panel for the floor lift was not removed. c. On the first floor bagging area of building 1 grinding, fugitive "Chlorez 700 S" dust accumulation of .525 to 2.75 inches which had covered the surface on the main grinding control panel was not removed. d. On the first floor bagging area of building 1 grinding, fugitive "Chlorez 700 S" dust accumulation of .224 to .613 inches which had covered the surface on the flaker panel was not removed. e.
