29 CFR 1910.36(b)(1):Two exit routes. At least two exit routes must be available in a workplace to permit prompt evacuation of employees and other building occupants during an emergency, except as allowed in paragraph (b)(3) of this section. The exit routes must be located as far away as practical from each other so that if one exit route is blocked by fire or smoke, employees can evacuate using the second exit route. a. On or about January 2, 2025, employees worked in building 46, performing required production and maintenance duties with only one exit route from work areas on elevated floors of the building. Normal production activities associated with the 9228 process include the use of flammable liquids, with potential generation of flammable vapor during releases from the covered process. Operators work throughout the second, third and fourth floors on a daily basis, with only a single stairway providing a pathway to the ground floor exits from building 46.

Employee training shall include at least: 29 CFR 1910.1200(h)(3)(ii): The physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area; a. On or about November 8, 2024, AP employees performing catalyst charging for the 56R vessel had not received specific hazard communication training for hazards applicable to the task. The 56R vessel was being brought back into service after having been emptied for repair. The employees had not been trained in the deflagration hazards associated with the static electricity ignition properties of the catalyst (Amberlyst 36) and were not aware that manually adding the catalyst into the vessel headspace posed risk for igniting flammable vapors from the vessel contents.
