29 CFR 1926.451(b)(1)(i):  Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    Santa Rosa Guard Station construction worksite, Andersen Air Force Base, Yigo, Guam  On or about July 31, 2012 and at times prior thereto, the upper tier of the scaffolding planks were 5 3/4 inches width between planks. Employees were exposed to a fall hazard of ranging from 10 to 12 feet.

29 CFR 1926.452(a)(4):  Diagonal bracing in both directions were not installed across the entire outside face of all double- and single-pole scaffolds.    Santa Rosa Guard Station construction worksite, Andersen Air Force Base, Yigo, Guam.  On or about July 31, 2012 and at times prior thereto, diagonal bracing in both directions were not installed across the entire outside face of the single-pole scaffold exposing workers to fall hazards ranging from 10 to 12 feet in height.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:                                                                     Santa Rosa Guard Station construction worksite, Andersen Air Force Base, Yigo, Guam.  Employees were working next to exposed unguarded rebar on scaffolding and on the guard station building at heights ranging from 10 to 12 feet.  Employees were exposed to serious impalement injuries.
