29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: Classroom Building #6, Andersen Air Force Base, Yigo, Guam. On or about July 31, 2012 and at times prior thereto, employees were working on top of a wooden frame building near exposed unguarded rebar at ground level.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. Classroom Building #6, Andersen Air Force Base, Yigo, Guam. On or about July 31, 2012 and at times prior thereto, the employer failed to have defective portable ladders either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. The ladders had missing rungs, steps, and other defective components that exposed workers to serious injuries from falls.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: Classroom Building #6, Andersen Air Force Base, Yigo, Guam. On or about July 31, 2012 and at times prior thereto, the employer failed to keep the worksite, passageways, ladders, and scaffolds cleared of debris exposing workers to slip, trip, and fall hazards.
