29 CFR 1926.451(g)(4)(iii):     The scaffold did not have a mid-rail as part of a fall protection system:      Job site located at 11335 S 204th, Gretna, NE 68028-Employees worked on a scaffold platform that was 82" above the ground level. The front of the scaffold work platform was 28" from the opening to an unprotected elevator shaft.  The ground level of the opening of the elevator shaft was 51" above the bottom of the elevator shaft.  The potential fall distance for employees working on the scaffold work platform next to the unprotected elevator shaft was over 11 feet to the bottom of the shaft.  The employees were not protected from falling to the bottom of the elevator shaft.
