29 CFR 1910.134(h)(2)(i): All respirators shall be stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they shall be packed or stored to prevent deformation of the facepiece and exhalation valve. Two Nova 2000 air-supplied respirator hoods were observed improperly stored and lying in the grass outside the water tower at 318 West Second Street, O'Fallon, IL upon inspection of the site.

29 CFR 1910.134(i)(7): For oil-lubricated compressors, the employer shall use a high-temperature or carbon monoxide alarm, or both, to monitor carbon monoxide levels. If only high-temperature alarms are used, the air supply shall be monitored at intervals sufficient to prevent carbon monoxide in the breathing air from exceeding 10 ppm. The Sunbelt Rental HP 915 compressor, s/n 333304, was not equipped with a high-temperature alarm or carbon monoxide alarm, and employees using air-line respirators connected to the compressor were subject to heat related and carbon monoxide exposure hazards.

The employer shall comply with any specific regulations that apply to work in dangerous or potentially dangerous areas. The employer failed to test the atmosphere of the confined space of the water tower located at 318 West Second Street, O'Fallon, IL with a calibrated direct-reading instrument prior to each entry by employees, subjecting employees to potential hazardous atmospheric hazards arising from lack of oxygen, high levels of combustible gases or high levels of carbon monoxide.
