29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about September 07, 2012, employees performing roofing activities inside a church steeple used a ladder that did not extend 3-feet above the upper surface.  Employees were exposed to a fall hazard.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On or about September 07, 2012, employees performing roofing activities on a church roof were not wearing fall protection or protected by other means of fall protection systems.  Employees were exposed to a hazard of falling approximately 28 feet to the ground below.    LON SMITH ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311954424, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 04/21/2010 WITH A FINAL ABATEMENT DATE OF 05/06/2010, WITH RESPECT TO A WORKPLACE LOCATED AT 4201 HERITAGE TRACE PARKWAY FORT WORTH, TEXAS.

Employees were exposed to a fall hazard.    A-1 SYSTEMS, INC - DBA: LON SMITH ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.1053(b)(4), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 310445002, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 03/03/2008 WITH A FINAL ABATEMENT DATE OF 02/11/2008, WITH RESPECT TO A WORKPLACE LOCATED AT 13600 NORTH FREEWAY, FORT WORTH, TX
