UHP Projects, Inc., overhead storage platform. On or about September 18, 2012, the second floor overhead platform used to store records, was not equipped with a standard guardrail system, consisting of a toprail at 42 inches and a mid-rail at 21 inches on all open sides of the platform, thereby exposing employees to a fall hazard greater than four feet.

On or about September 18, 2012, the employer did not ensure the eye wash station available for use by employees was in good working order, in that there was not an established schedule for maintaining or flushing the unit as evidenced by the dust/debris covering the nozzles and the bowl.
