29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average (TWA) of 85 decibels : (a) On or about November 2, 2012 - Packaging, an employee was exposed to a continuous noise level at 75.27% of the permissible daily noise exposure (8-hour time-weighted average sound level of 90 dBA or a dose of 100%) during the 441 minute sampling period on November 9, 2012. Exposure calculations include a zero increment for the 39 minutes not sampled.
