29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. On or about January 24, 2024, and at times prior thereto, employees were operating extruder machines on a wet walking-working surface, exposing the employees to a slip hazard.

29 CFR 1910.303(g)(2): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures. On or about January 24, 2024, and at times prior thereto, an employee was performing administrative duties in the shipping and receiving office that had an open electrical panel with live electrical conductors in excess of 120-volts, exposing the employee to an electrical hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about January 24, 2024, and at times prior thereto, employees were dumping material into the regrind bin that had a damaged electrical cord, exposing the employees to an electrical hazard.
