29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units: On or about December 13, 2012, employees who were working on and around a fabricated frame scaffold that had hollow-core masonry units being used in four locations as footings were exposed to scaffold displacement hazards while constructing masonry block wall at the above job site.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about December 13, 2012, employees who were working on a fabricated frame scaffold that was lacking guardrails, without means of fall protection were exposed to a fall hazard of approximately 20 feet while constructing masonry block wall at the above job site.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. On or about December 13, 2012, the employer did not provide training to employees who had performed work on a scaffold and were exposed to a fall hazard of approximately 20-feet, that enabled them recognize the nature of fall hazards and to understand the procedures to control or minimize those hazards at the above job site.
