OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees were exposed to inhalation hazards from hydrogen sulfide (H2S) gas. On or about January 4, 2013, and times prior thereto, the employer did not provide hydrogen sulfide (H2S) training to the sand operators and sand pushers when working in and around H2S gas. The employer did not provide any H2S monitoring equipment to the sand drivers and sand pushers when working in around areas where there is a potential for exposure to H2S gas. The employer did not conduct periodic testing of the H2S monitors that were provided to the water drivers.
