29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board.    a) Press 1 - Catwalks at the second and third levels have gaps that are approximately 21 inches wide adjacent to the dryers. Employees are exposed to fall hazards of 8 feet from the second level and 16 feet from the third level to the concrete floor below on or about February 14, 2013.    b) Press 2 - Catwalks at the second and third levels have gaps that are approximately 21 inches wide adjacent to the dryers. Employees are exposed to fall hazards of 8 feet from the second level and 16 feet from the third level to the concrete floor below, an employee fell through the gap on or about February 17, 2013.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.28(a)(1):  Scaffolds were not furnished and erected in accordance with this standard for persons engaged in work that could not be done safely from the ground or from solid construction.    a) Press 1 -  The employer did not furnish or erect scaffolds for employees to use when they entered the dryer gaps to perform work. The employees walked on 1.5 inch round bars and/or 3 to 4 inch wide "I" beams and were exposed to fall hazards of 8 feet from the second level and 16 feet from the third level to the concrete floor below on or about February 14, 2013.    b) Press 2 -  The employer did not furnish or erect scaffolds for employees to use when they entered the dryer gaps to perform work. The employees walked on 1.5 inch round bars and/or 3 to 4 inch wide "I" beams and were exposed to fall hazards of 8 feet from the second level and 16 feet from the third level to the concrete floor below, an employee fell in the gap at zone 2 of dryer #7 on or about January 17, 2013.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
