On or about 2/13/13, at the warehouse area of the above referenced worksite.  An employee was exposed to an electrical hazard while working with a fan (Air Master BTA48R8/6) that lacked a ground prong on its attachment plug.

29 CFR 1910.305(b)(2):  Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose    a.  On or about 2/13/13, at the warehouse area of the above referenced worksite.  An employee was exposed to an electrical hazard while operating a toggle switch which was installed within a junction box that lacked a cover plate.

29 CFR 1910.23(c)(1):  Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:      a.  On or about 2/13/13, at line #2, palletizer area of the above referenced worksite.  An employee was exposed to the hazard of falling from a height 8'-6" while working standing on a platform which had an incomplete guardrail.      Shasta Beverages Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 312154859, citation number 2, item number 1 and was affirmed as a final order on 6/03/09, with respect to a workplace located at 3550 NW 110th St, Miami, FL 33167.
