(a) Surimi Processing Area:  On or about 14 February 2013 and times prior thereto, an open hatch measuring approximately two foot by two-foot five inches was open without railings installed.  This condition exposed employees to a fall hazard of approximately 15 feet-11 inches to the lower deck surface.      (b) Fish processing room, Lower deck access hatch:  On or about 14 February 2013 and times prior thereto, an open hatch measuring approximately two foot by two foot-five and half inches was open with only a loose chain rail installed.  This condition exposed employees to a fall hazard of approximately 15 feet-11 inches to the lower deck surface.

29 CFR 1910.151(c):     Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.    Fish Processing Area:  On or about 14 February 2013 and times prior thereto, only one eye wash station was established on the port side of the factory for fifteen minutes of continuous flushing of the eyes.  This condition exposed employees to chemical hazards in the event of exposure to corrosives on the starboard side of the vessel, due to limitations in access and time to get to the other side of the vessel.
