(b) C/P Arctic Fjord (Surimi Wash Tanks):  On or about 2/14/2013, and times prior thereto, affected employees were not aware of the of the de-energization of the Surimi Wash Tanks, as they were not tagged nor identified as being locked out on the machine or in a visible location; exposing employees to the release of hazardous energy.
