29 CFR 1910.37(a)(4):     Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.    Main Shop:  On or about 15 February 2013 and times prior thereto, 2 emergency exit lighting were inoperative.  This condition would expose employees to delays in exiting the workplace, smoke inhalation, and fire hazards in the event of an emergency.

29 CFR 1915.74(b):     Gangways meeting the requirements of 29 CFR 1915.24(a)(1), (2), (9), (10), and (11) were not provided for access from wingwall to vessel, or were not provided when two or more vessels, other than barges or river towboats, were lying abreast.    Gangway to Drydock:  on or about 15 February 2013, and times prior thereto, there was a 51 inch gap between the uprights on the top of the gangway railing where it had been damaged; exposing employees to a fall hazard into the water.
