29 CFR 1910.305(b)(2):     Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose.    Main Shop, Starboard side:  On or about 15 February 2013 and times prior thereto, the hydraulic test machine control box had a knock out measuring approximately 1 and 1/4 inches diameter.  This condition exposed employees to a 480 volt electrical shock and burn hazard.

29 CFR 1915.56(b)(2):     Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected.    Javeh Jireh, Main shop area:  On or about 15 February 2013 and times prior thereto, the Migmaster 250 plasma welder, set at 250 amps, had exposed lead wires approximately 51 inches from the stinger end.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1915.132(a):     The frames of portable electric tools and appliances were not grounded through a third wire in the conductors, or through a separate wire which was grounded at the source of the current.    (a) Javeh Jireh, Tool Room:  On or about 15 February 2013 and times prior thereto, the Milwaukee band saw, Model Number 6230N, Serial Number C05AD09010871, was missing a ground pin.  This condition exposed employees to an electrical shock and burn hazard.      (b) Javeh Jireh, Tool Room:  On or about 15 February 2013 and times prior thereto, the Milwaukee screw shooter, Serial Number 0464246926, was missing a ground pin.  This condition exposed employees to an electrical shock and burn hazard.
