29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Maintenance Shop:  The Eisen/Bridgeport Mill did not have a guard to cover the unused portion of the bit and chuck above the material being worked.     (b) Maintenance Shop: The Progress Drill Press did not have a guard to cover the unused portion of the bit and chuck above the material being worked.    (c) Maintenance Shop: The Powermatic Drill Press did not have a guard to cover the unused portion of the bit and chuck above the material being worked.    (d) Maintenance Shop: The Dormac 180S lathe did not have a chuck guard.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    (a) The three Die Carts by the USI Press 523, did not have adequate strain relief as where the electrical wiring was attached the outer insulation was broken exposing interior wiring.    (b) Forming Deparment: There was not adequate strain relief for the 2 hand control for the AIDA 2000, Machine # 506, as where the electrical wiring connected to the press the outer insulation was broken exposing interior wiring.
