29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) The three Die Carts by the USI Press 523, did not have adequate strain relief as where the electrical wiring was attached the outer insulation was broken exposing interior wiring. (b) Forming Deparment: There was not adequate strain relief for the 2 hand control for the AIDA 2000, Machine # 506, as where the electrical wiring connected to the press the outer insulation was broken exposing interior wiring.
