29 CFR 1910.215(a)(4): Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. a) At the maintenance shop - Employees were using a "Jet" (Model: JBG-10A; SN: 12020030; 60 Hz, 10 Amp, 1750 rpm) pedestal bench grinder to grind steel parts, the work rest opening was one-fourth inch.

29 CFR 1910.215(b)(9): The distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch. a) At the maintenance shop - Employees were using a "Jet" (Model: JBG-10A; SN: 12020030; 60 Hz, 10 Amp, 1750 rpm) pedestal bench grinder for grinding steel parts that had a distance of one-half inch between the abrasive wheel periphery and the adjustable tongue guard. Blount International, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 892385, citation number 1, item number 4 and was affirmed as a final order on 04/02/2013, with respect to a workplace located at 4840 East 12th Street, Kansas City, MO 64127.
