29 CFR 1915.81(b)(1)(ii): The employer did not ensure that each walkway was clear of tools, materials, equipment, and other objects that create a hazard for employees: a. at the main entrance way decking, metal flashing strips debris was left piled in the walkway during idle time, posing a tripping hazard b. along the main entrance corridor, near the section AFF3/AFF4, a flexible white rod was dangled in the walking path at employee face level, posing a laceration and impalement hazard c. adjacent to the walk-in deck area, two raised floor flaps posed a tripping hazard next to a vessel elevator shaft with a sagging webbing guard wall.

29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE: On this vessel, at the weather deck, employees performing materials and liquids removal were exposed to lower levels exceeding 5 feet while working without a personal fall arrest system (PFAS): a. at the water side, a section next to equipment being disassembled, was missing a proper height midrail b. at the middle, an existing elevator opening with a fall hazard exeeding 15 feet to the lower deck level, was only provided with a top rail cable.
