29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE:    On this vessel, at the weather deck, employees performing materials (insulation) and liquids removal were exposed to falls to lower levels exceeding 5 feet while working without a personal fall arrest system (PFAS):    a.  at the weather deck, guardrail cables were secured with less than the recommended minimum of three Crosby clips and the clips were attached in reverse of the recommended arrangement    b.  at the weather deck, where only a top guardrail cable and no midrail cable were provided at the edge of the working surface.
