On or about February 27, 2013, personnel helping with and working next to the arc welding operations were not utilizing shaded eye protection.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts:  a) Lake Street facility, roll forms area, west end of building.  On or about February 27, 2013, the flat, metal rolls rotated when feeding into the forming machine.  The rolls were not guard to protect employees from being caught by the rotating roll or the ingoing nip-point created when the rolls are rotating.  b) Lake Street facility, roll forms area, Pearson Beam Line.  On or about February 27, 2013, the ingoing nip point on the Herr-Voss straightener was not guarded to prevent employees from placing their hands/fingers in the nip point.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:  a) Lake St. facility, packaging and shipping area.  On or about February 27, 2013, the sawdust found in the area around and below the Delta wood-cutting saw was not cleaned on a regular schedule and was allowed to accumulate.
