29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing on all open sides:   a) Stateline facility, floor level where pallets are stored, next to the stairs to the production area.  On or about February 26, 2013, the gate, guarding the open side of the elevated floor, was not closed.  Personnel were exposed to a fall of 5 feet to the lower level when using the stairs to go between the floor levels.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes was not provided within the work area for immediate emergency use:  a) Stateline facility, north end of building, on wall of parts washer in the production area.  On or about February 26, 2013, personnel worked with corrosive materials including, but not limited to, Chem Add 6310, Sealtex 1523NCS and Enprox 714, and the eye wash station was not maintained in that, the it was dirty and it spewed brown water when first activated.    b) Stateline facility, north end of building, on wall of parts washer in the production area.  On or about February 26, 2013, personnel worked with corrosive materials including, but not limited to, Chem Add 6310, Sealtex 1523NCS and Enprox 714, and the eye wash station was not inspected on a weekly basis.

On or about February 26, 2013, the flat, metal rolls rotated when feeding into the forming machine.  The rolls were not guard to protect employees from being caught by the rotating roll or the ingoing nip-point created when the rolls are rotating.

Stateline Facility, Front Entrance to Building/Near Parking Area -- Natural Gas Main Located Between The Building And Parking Lot -- On or about February 26, 2013, personnel were potentially exposed to fire/explosion hazards from natural gas mains and associated piping that were not protected against physical damage.  A section of the natural gas main and its associated piping were not protected from being accidentally struck-by vehicles.

On or about February 26, 2013, personnel conducting welding operations were using welding jackets and protective clothing that was not maintained in reliable condition in that, the jackets/clothing had burn holes and tape was utilized to patch rips and holes in the protective equipment.
